Maintaining trust

  • time, location, and date of the meeting
  • name and roles of those present
  • facts presented in the discussion
  • participants' reactions and responses
  • agreed-upon next steps
  • any plan for providing follow-up and further information to the patient and family, if appropriate
  • name and details of the patient's contact person
