Introduction

Informed discharge is the dialogue with a patient that alerts them to the symptoms and signs that would indicate a need to seek further medical care. The discussion is a fine balance between alarming patients and giving them a false sense of security.

Adequate discharge strategies help reduce the risk of complications for patients and also unnecessary office visits, unplanned hospital readmissions, unfavourable patient outcomes, and medico-legal problems for physicians.
