Transferring care to others

Documentation of handovers

Important information can be lost when multiple handovers occur. If you are assuming care of a patient, it may be prudent to reconfirm the clinical history directly with the patient, and then to enter key elements of handover information in a paper or electronic log to aid your memory.

A formal transfer of responsibility for care, for example, from one clinical service to another, should be recorded in the patient's medical record.
